The construction that means is used when you want to explain something/give the reason for something as in:

I want to get good marks in my exam and that means I must study very hard.

Hi Luby,

I explained 'that means' above. 'Which means' would not usually be preceded by 'and'.

You would say:

I want to get good marks in my exam, which means ....
