Do you find the following OK? Any suggestions would be greatly appreciated.

With reference to Mr. Proctor’s email regarding ‘the detailed appraisal of the book list’, could you please confirm with him if there is any specific format for it or could we design one for the same?

PS: I was a bit doubtful about the use of the second 'could'.

My suggestion:

With reference to ... regarding (ABOUT) the detailed appraisal of the book list, could you please (WOULD YOU KINDLY) confirm with him if there is any specific format (REQUIRED) or it or (ALTERNATIVELY) could we (WOULD WE BE ABLE TO) design one for (IT) the same?

Just refinements really, rather than corrections.
