Hi Tom

How about screechy, shrill or off-key?

Hi Tom,

I know exactly what you mean. I would suggest jarring or tuneless.

A voice can grate on your nerves in the sense that it has an unpleasant effect on your nerves. From that you can get 'a grating voice'.

I'd go for "lousy" since the example is a conversational piece.

.
Along the same lines as diverhank, you could also say crummy, rotten, or dreadful. They can all be used to mean "terrible".
