The main difference is that one is the subject (who) and one is the object (whom). Take these two examples:

That's the man who (subject) spoke to me yesterday

That's the man whom (object) I (subject) met yesterday.

If you break these examples down into separate sentences, you have:

That's the man. He (subject) spoke to me yesterday

That's the man. I met him (object) yesterday
