It is past tense: "Samantha broke her promise". If you wish to use present tense, you must use 3rd person "breaks": "Samantha breaks her promises."

Could you please explain why the first sentence is past tense while the second one is present?

You mean the clauses in the test sentence? There is no rule that verb tenses must remain the same within a sentence; in fact, it is impossible to do so when the sentence speaks of various events at various times:

I had breakfast at 6:00 this morning, I'm having lunch now at noon, and I'll have dinner at 7 tonight.

The verbs in the test sentence reflect the different times of the breaking and the trusting.
