It seems there are no correct ones there. It should be like this: "She asked me where I stayed when I visited Toronto".

Hi,
and with your corrections, can we accept the 3rd and the 4th?
She asked me where I was staying while visiting Toronto.
She asked me where to stay when visiting Toronto.

And should we use Sequence of Tenses to express a prior action?

These are my first impressions:

1. She asked me where I stayed when I visited Toronto.

— e.g. she wants the name of the hotel, or the location.

2. She asked me where I had stayed when I visited Toronto.

— she thinks you may have stayed at more than one location.

3. She asked me where I was staying while visiting Toronto.

— she is referring to a future trip.

4. She asked me where to stay when visiting Toronto.

— she wants advice.

(But those are only first impressions: with a full context, the interpretations might be different.)

Best wishes,

MrP

MrP's paraphrase, though of his first impression, is across the board, I think.
