If I went to a clinic and I wanted to know whether the health insurance that I have is valid to use, can I use the above sentence?

Hi si,

'Do you accept my health insurance card?' is what I'd expect in this situation. You could also point at your card and say 'Am I covered?'

I like your version better, but if I want to use the verb "honour" here, can it only be used in passive form?

The health insurance is honoured here.
