Hi Tom
Maybe it would be easier to look at "each and every" as a fixed collocation meaning "every single". Thus:

Horrifying, isn't it? :lol:
Torsten mentioned a new search engine named "Cuil" the other day. Interestingly, Cuil doesn't provide a single example of "each and everything" on the first page of its results. All of the results on the first page are simply "everything" or "and everything": http://www.cuil.com/search?q=%22each+and+everything%22
