OR

Sounds OK to me. This one suggests the current state of the problems. I could be wrong here though.

These problems were reported in previous versions of this application"

Look at it as the passive voice of, "They reported these problems in previous versions of this application"

As for the second question,

This application never workS properly . This is correct. application is a singular noun
