I googled a word 'innocuous' and found this example sentence, and I have no idea what this means.

An innocuous challenge by Moses saw a penalty awarded which Andy Stephens could do nothing about.

What does 'saw' mean in this sentence?

Hi,

The verb 'see' (in your sentence the past form 'saw' is used) can mean 'revealed' , 'showed' or 'result in'. To explain the sentence: A harmless challenge by Moses resulted in a penalty (a fine/punishment) being given out which Andy Stephens couldn't stop/prevent.
