'Evaluation' takes place prior to the 'result', I suppose.

a result (noun) - is an outcome or consequence, evaluation is the determination of the value, significance, worth, or condition
First we get the result then we evaluate its value.

Quote:
Feedback is important to provide continuous evaluation of outcomes to see if plans are being achieved

In your example the sequence is: 1. outcomes (results)
2. feedback (the return of information about the result)
3. evaluations of the results in order to see if plans are being achieved

If you take an exam; your teacher grades it (a result ) (1), you receive your grade from your teacher (a feedback) (2), and then your see if you have passed or failed (an evaluation). (3)
