Feedback is important to provide/for providing(both are correct) continuous evaluation of outcomes to see if plans are being achieved.

Please somebody cross check my answer and tell me if I am wrong.

Arun is right. "For" is never used before an infinitive; just as "to" is not regularly used before a gerund except in different constructions (such as "this amounts to allowing the natives full freedom", where "to" is used only because of "amounts").
