Where is a mistake ? Is there a spelling mistake or not?
My Microsoft Word did not find any spelling mistakes in your sentence, although we usually say "the patient was lucid," not his mind.

There was a little word order mistake. " Was the patient lucid during the evaluation?"

I'm sorry for misrepresentation but I have this one in closed test. And the question has been "Find a mistake". I was also surprised and hadn't any idea what the mistake there is. Maybe the test is mistake per se.
