I would like to appear for TOFEL from UK

I am a doctor originally from India but now in UK . in progress of taking up post graduation. I would like to appear for TOFEL from UK. Can anyone let me know about details of this examination, which are as follows:

- Books needed or any useful website.
- Availibilty of application form where in UK[ London mainly]? when? fees?
