One meaning this sentence can have is:
This instrument was created heat-resistent.
It was heat-resistant from day one.

Could it ever mean that it wasn't heat-resistant at first and was modified to become heat-resistant?

I would say your sentence - This instrument was made to be heat-resistant means that the instrument was made/produced in such a way that it would resist heat.
