In what context, this can be asked?

It does not mean much without context. It depends on what "it" is.
It might mean do not turn it on (Keep the lights off.), or do not put it on (Keep your coat off, it is too hot.), or something entirely different.

There is NO context, in which this can be asked, because "keep it of' is not a question.

Let me give an example below:
1. Very few people succeed in losing weight and -keeping it off-.
What does the phrase "keeping it off" mean?
Here What "it" refers to? & "off" refers to?
Please be kind enough to explain.

In this case "keeping it (weight) off means that the lost weight is not gained again.
It is very difficult to lose weight and become slim, it is even more difficult to stay slim and not to gain the weight back. Very few people succeed in doing it.
