This sentence comes from the Longman Dictionary of Contemporary English:

What does it mean?

Thanks

Ms/Miss/Mrs/Mr - Please delete as applicable.

This is an instruction you would find on an application form, for instance. You are to strike through the three titles that do not apply to you, leaving the appropriate title unmarked.

As Mister Micawber, I would put a line through 'Ms/Miss/Mrs/', leaving my title as 'Mr'.
