"What has happened?"

"What's your name"? "What is your name?"
"What's he said?" "What has he said?"

"What is happening?"
"What has happened?"
I thing it depends on time you want to describe. If you mean action in the past you should use has.
====
"What's your name"? "What is your name?"
"What's he said?" "What has he said?"
It is the same expression. Just in shot form.
