We are always asked the nationality during form filling. My english teacher teaches me that we have to use adj. For example,

Q.7 The nationality of the passport that you currently hold. For example if
you hold a Chinese passport then write "Chinese".

But recently I saw a passport of a US citizen. On his passport, the nationality is USA, not American!

So I am confused. So when asked during filling a form, what (noun, or adj) does one use?

If they ask for nationality write like 'American' 'Canadian' etc.
But if they ask for country write the name of the country.But to be honest I dont know about it,I am just giving you an idea,but the question.

I have looked in my own passport. It does not ask for a nationality.
It does ask for a place of birth. Which you have to answer with a name of the country or city of your birth (a noun).

another intersting thing:

Does "American" also refer people in Canada, Mexico, and etc?

The passport asks for both.

I disagree because, for example, i'm jew and i'm russian citizen (have a passport of The Russia Federation), and it is written jew in the nationality sectioninside.
