Which, if any, of the following would be correct as far as the neither / nor usage ?

1) The information is therefore neither reflected in my application nor formal resume.

2) The information is therefore neither reflected in my application nor my formal resume.

3) The information is therefore neither reflected in my application, nor my formal resume.

May I ask why not include the "therefore"? The sentence was the last in a paragraph, which explained having accepted a temporary position immediately after completing an application and before it's mailing.

The sentence I ended up using was, "The information is therefore reflected in neither my application nor my formal resume."

Hopefully it will read well, and not raise any red flags.
