I always a little hesitate when I want to use "a same..." insted of "the same". Could you tell me what the right time is for using "a same..."?

Usually, you should use 'the same'.

That should be 'I can't let the same one occur...' as far as I can see.

I'd wonder if "the same one" sounds too definite as if the doctor knew which one could while "a same one" suggests whichever one as long as it's the same. But I may be very wrong.

I think I understand what you're getting it, and can appreciate the logic behind it, but all the same I imagine that this would be a very rare occurrence.
