Hello Alan, Mister Micawber, Beeesneees, Mordant, Esl_Expert and other native English speakers,

-------------------------------
On a scale of 1 to 5—1 being mild pain and 5 being extreme pain—tell me how much pain you are in.
-------------------------------

Can I say "feel" or "have" to mean are in? Do you think the original sounds better?

Yes, but the original is a natural utterance.
