Doctor: Good morning Mr.Wilson. What can I do for you?
Patient: I've had a sever headache for several days and I fell completely exausted.
Doctor: Do you have nausea or vomiting?
Patient: No.
Doctor: How's your appetite been?
Patient: Very poor but I seem to be thirsty all the time.
Doctor: Have you ever felt dizzy?
Patient: No.
Doctor: Do you get tired easily?
Patient: I often get breathless without any obvious reason.
Doctor: How long have you been feeling like this?
Patient: I don't know exactly but I think for about a month.
Doctor: Do you smoke?
Patient: Yes, but only 5 or 6 cigarettes a day.
Doctor: Are you fond of alcoholic drinks?
Patient: Oh no. In fact I never drink alcohol, can't even stand the smell of it.
Doctor: All right. We shall examine you now and see how we can help you.

Even if it looks simple it really isn't to me :( ... any help appreciated

I am positive this is direct speech ( I hope) it needs to be put into indirect one... I know that mostly it is time reverse but what with No or when I have 2 verbs in sentence. I completely suck at this.

I would place the definite article before 'doctor' and 'patient' each time, but the direct to indirect speech aspects are good.

I don't know what the expectations are in terms of varying the language patterns used to introduce the speech ( xasked, x replied, x said is repetitive) but there's nothing wrong with the reporting of indirect speech, other than that you say 'alcohol drinks' instead of 'alcoholic drinks'.
