The fourth of the twelfth two-thousand and twelve
The fourth of December two thousand and twelve.
The thirteenth of the eleventh two thousand and eleven.
The thirteenth of November two thousand and eleven.
... those are just some of the possibilities.

etc. is the shortened form of 'etcetera' ate - set - err - ah.

Have you tried looking at the definition of 'fade' in a dictionary?

Does that make it clear?

It has 2 main definitions, depending on context:

1) becoming paler or less bright
2) disappearing gradually

It has a number of other less common definitions too.
