1. "I can't think of it right off hand."
2. "I can't think of it right off head."
Do these two sentences mean the same?

Your second expression needs a change:
I can't think of it (right) off-hand (without further thought or consideration)
I can't think of it (right) off the top of my head (from quick recollection, without further thought or consideration)

I would change these to:

1 I can't think of it just like that/on the spot.

2 I can't think of it off the top of my head.
