Please help me I'm lost with this little question.

Exercise A: Write a form of the word in the bracket in each gap to complete the sentences.

Q 1 His brother hit him on the head. (hit)

My question is do we need to add s on hit, please help.

That sentence is acceptable as it is. It is written in the past tense.

To change it to present tense, you have to add an 's': His brother hits him on the head.

Unless you've been told to change it from past tense or to change it to present tense, the question is flawed.
