Is that a formal letter?
The specific expressions and words depend on what you want to say.
The standard formal layout would be:
Write your address in the top right corner of the paper.
The address of the Institute might be added on the left hand side of the page under your address. (This can be left out if the letter is slightly less formal.)
The date should come underneath your address on the right.
Begin your letter with Dear ..., (If you don't know the name of the recipient, write 'Dear Sir')
End with 'Yours faithfully,' (very formal) or 'Yours sincerely' (slightly less formal)
