1. To him I made a request which he did not comply.
2. I made a request to him, which he did not comply.
3. To him I made a request with which he did not comply.
4. To him I made a request for which he did not comply.
5. I made a request to him that he did not comply.
Please correct the above sentences.

They should all be corrected to these:

I made a request of / submitted a request to him, with which he did not comply.
I made a request of / submitted a request to him, which he did not comply with.

I suppose some would write that, yes. The BBI does not list that collocation.

Oh, I don't know— this is muddy area. Perhaps:

He made a formal request for action by the police in the matter.
