Why we choose "what" for that blank?

It's a poor sentence to begin with, but I would use 'those that' there. I would not use 'what' or 'that' (on its own).
Meals in Spain are quite different from those that they have in China.

I would probably use one of these before I used the above though:
Meals in Spain are quite different from the food they have in China.
Meals in Spain are quite different from the meals they have in China.
Meals in Spain are quite different from the ones they have in China.

"What" is the correct answer in my exam. So , OK. thank you.

In passing, can we put "the" before "meals" in the three sentences above?
If so, is including the article optional?

You can put 'the; in where you suggest, and obviously then it's optional at the beginning. It is more difficult to remove the article before the second set of food/meals.
