5a. They decided to consult with a doctor. (OK)
7. It was very late in the night and the streets were empty. (Not usual but possible.)

#1 has no object for one of its transitive verbs. #4 and #5 have the wrong verb form. #6 verb is not wrong, but it is only for limited circumstances. Please fix the 4 problems and re-post.

#1: I told you that #1 needs an object, not a verb change.
#4: Notice the durational phrase 'for the last 3 weeks'. That is critical to verb choice.
#5: The phrase 'every year' is critical to verb choice.
#6: You have not tried to fix it yet.

#1- Please read my original comment about #1 CAREFULLY.
#4 - Good!
#5 and #6 are now both 'correct' in that they can be used in limited circumstances (i.w. with certain further context). However, both are 'wrong' in that they are the expected, usual verb form in these cases. Please try again. Do not be lazy and accept any answer.

#1-- You must add an object to the other verb in the sentence.
#5 and #6 both require simple present verb forms because they refer to a repeated action and a condition respectively.
