Certified vs. to certify vs. to be certified

can u please tell me the difference:

1. it is certified that Mr. Johan has worked....

2. it is to certify that Mr.........

3. it is to be certified that Mr.....

If it's already a complete action that took place in the past you should use
it is certified that Mr. Johan has worked...

your second sentence is not correct

and the third implies that the fact that Mr.Johan has worked must be certified.
Maybe I'm wrong
